John doesn't approach. It's not... wise. Perhaps it's not kind, but neither is the idea of any lingering effect from whatever the fuck caused earlier prompting Arthur to take that knife and plunge it into him. And given Arthur's anguish over the bite, he doubts he would be able to handle that regardless of whether or not John was there to talk him down from it. So instead, he settles where he is.
